All California schools and community centers are eligible for Tooth Mobile services. Find out how to arrange a visit below.

School administrators have reported that, as Tooth Mobile dentists relieve their students' dental pain, self-confidence and motivation increases. Improved classroom performance and increased attendance rates then follow. To arrange a Tooth Mobile visit, please follow the instructions below. The process is simple and efficient, as we are anxious to bring our program to you.

Step #1 Contact Patient Relations at Tooth Mobile, at (408) 244-6029 to schedule a visit.

Step #2 Download the Application and Consent Form and the Patient Medical History Form (available in English, Spanish and Korean). Photocopy and distribute both forms to all students. Application and Consent Form [Spanish] [Korean] Patient Medical History [Spanish] [Korean]

Step #3 Fax the completed Application and Consent Forms to Tooth Mobile at (408) 244-9553, Or you can mail them to Tooth Mobile, 1659 Scott Blvd. Suite 4, Santa Clara, CA 95050.

Tooth Mobile has achieved more success in its overall goal of improving oral health when schools designate the week coinciding with the Tooth Mobile visit(s) as "Oral Health Week." We encourage each school to contact Colgate to receive its packet of educational materials, including videos and activities to be used during that week.
